• Applicants must reside in the City of Corning.
• Completion of an application form is required by both the applicant and his/her attending physician.
• Either a temporary or permanent permit may be obtained, according to the direction of the attending physician.
• Permanent permits are valid for two years, and are renewable in two year increments.
